- When you sign up, you will automatically download our Dog Health Assessment intake form. You can choose either the PDF or the Word version to fill out, whichever is easiest for you.

- Please also contact your local veterinarian and have them send the last two years of veterinary records to you.
- Send the completed Dog Health Assessment and your pet's veterinary records to Dr. Kevin. His email address is DrKevin@TheLongevityVet.com
- You will hear from Dr. Kevin within one working day, and he will complete his initial Consult report within two days.
- He will then email you that report, and we can then set up a Zoom call to discuss your pet's custom longevity strategy with you
