What Is Feline Chronic Kidney Disease?
Chronic kidney disease means a cat's kidneys have lost function gradually, over months or years, and that loss is generally permanent. The kidneys filter waste products from the blood, regulate fluid and electrolyte balance, help control blood pressure, and produce hormones involved in red blood cell production. As functioning kidney tissue is progressively lost, waste products build up in the blood (a state called azotemia) and the kidneys lose their ability to concentrate urine.
CKD is extremely common in cats — studies estimate a large share of cats over 15 have some degree of it — which is exactly why proactive screening in middle-aged and senior cats matters as much as treatment after diagnosis. In most cases the underlying cause is never identified (it's simply labeled "chronic interstitial nephritis" or age-related nephron loss), though specific causes like polycystic kidney disease, chronic dental/periodontal disease, hypertension, and prior acute kidney injury are recognized contributors.
Symptoms of CKD in Cats
Like HCM, early CKD is often silent — a cat can lose a significant amount of kidney function before obvious signs appear, since the kidneys have considerable reserve capacity.
Early or subtle signs
- Increased thirst and urination (often the first sign owners notice)
- Mild weight loss despite a normal appetite
- Coat becoming dull or unkempt
- Slightly reduced energy
More advanced signs
- Poor appetite or selective eating
- Vomiting, especially in the morning
- Noticeable weight and muscle loss
- Bad breath with a distinct ammonia-like or "uremic" odor
- Lethargy, weakness, or hiding more than usual
- Constipation
Because CKD is progressive and largely silent early on, routine bloodwork in cats over 7 — not waiting for symptoms — is the single best tool for catching it while there's the most opportunity to slow it down.
Risk Factors and Contributing Causes
- Age — the single biggest risk factor; prevalence rises sharply after age 10
- Dental and periodontal disease — chronic oral infection and inflammation is one of the most under-recognized contributors to declining kidney function in cats; untreated dental disease allows a steady low-grade bacterial and inflammatory burden that the kidneys must filter
- Hypertension (high blood pressure) — both a cause and a consequence of CKD; the two feed each other, which is why blood pressure is checked at every CKD-related visit
- Prior acute kidney injury — from toxins (lily ingestion, certain medications), dehydration, or infection, even if the cat appeared to recover fully
- Polycystic kidney disease — an inherited condition, especially in Persians and related breeds
- Breed — Persian, Himalayan, Maine Coon, Abyssinian, Siamese, and Russian Blue cats are reported at somewhat higher risk
How Is CKD Diagnosed?
CKD is diagnosed and staged using a combination of blood and urine tests, ideally run more than once to confirm persistence:
- Creatinine — the traditional core marker of kidney filtration function, though it doesn't rise until roughly 65–75% of kidney function is already lost
- SDMA (symmetric dimethylarginine) — a more sensitive biomarker that can flag declining kidney function earlier than creatinine, often before a cat becomes clinically azotemic — one of the more meaningful diagnostic advances in feline CKD in recent years
- Urine specific gravity (USG) — measures how well the kidneys can concentrate urine; dilute urine alongside rising creatinine/SDMA supports a CKD diagnosis over other causes
- Urine protein:creatinine ratio (UPC) — screens for proteinuria, which both indicates kidney damage and independently predicts faster progression
- Blood pressure — hypertension changes both staging and treatment
- Blood phosphorus and potassium — guide dietary and medication decisions and are tracked at every stage
Stages of Feline CKD (IRIS Staging System)
The International Renal Interest Society (IRIS) staging system is the internationally recognized framework for classifying CKD severity in cats and guiding treatment. Staging is based primarily on blood creatinine and/or SDMA, then further refined ("substaged") based on proteinuria and blood pressure.

Each stage is further substaged by proteinuria (non-proteinuric, borderline, or proteinuric) and blood pressure risk (normotensive through severely hypertensive), both of which affect the treatment plan independently of stage.
Treatment Options by Stage
Feline CKD treatment is not one prescription — it's a set of interventions layered in based on stage, substage, and the individual cat's bloodwork:
- Renal therapeutic diet — restricted, high-quality protein and reduced phosphorus; one of the most evidence-supported interventions for slowing CKD progression, generally introduced from stage 2 onward
- Phosphate binders — added when dietary phosphorus restriction alone isn't enough to control blood phosphorus, typically stage 3–4
- ACE inhibitors or angiotensin receptor blockers — used for proteinuria and/or hypertension management
- Antihypertensive medication (e.g., amlodipine) — for cats with confirmed hypertension, to protect the kidneys, eyes, heart, and brain from pressure-related damage
- Potassium supplementation — for cats with low blood potassium, common in more advanced CKD
- Subcutaneous fluids — used in stage 3–4 to support hydration and help flush waste products
- Appetite stimulants and anti-nausea medication — to maintain nutrition and quality of life, especially in stage 3–4
Emerging and Longevity-Focused Treatments
Rapamycin and Felycin-CA1
Rapamycin, an mTOR inhibitor, has research interest in CKD for its potential to modulate the cellular aging and fibrotic pathways implicated in progressive kidney decline — building on longevity research originally done in the context of aging and organ function more broadly. Felycin-CA1, our cat-specific rapamycin formulation, is used within our practice as part of a broader longevity-oriented protocol, alongside — not instead of — standard CKD staging and monitoring. The AIM protein ("FeliAIM")
One of the more scientifically significant developments in feline kidney research in the past decade is the discovery, led by Dr. Toru Miyazaki's laboratory at the University of Tokyo, that cats carry a species-specific defect in a blood protein called AIM (apoptosis inhibitor of macrophage). In most mammals, AIM helps clear cellular debris from the kidneys; in cats, AIM binds too tightly to circulating antibodies and can't do this job effectively, which is theorized to be a major reason cats are so disproportionately prone to CKD. Recombinant AIM protein therapy has shown improved survival and stabilized kidney biomarkers in feline clinical studies, and as of mid-2026 a commercial AIM-based drug has been filed for regulatory approval in Japan. It is not yet approved or commercially available in the United States. We're tracking this closely and will update this page as it develops — it represents one of the first treatments aimed at a root biological driver of feline CKD rather than only its downstream effects.
SDMA-based early detection
As referenced above, SDMA testing allows detection of declining kidney function well before creatinine rises, which meaningfully shifts CKD management earlier — from reacting to stage 3 symptoms toward intervening at stage 1–2, when there's more kidney function left to protect.
Monitoring Schedule
Monitoring frequency should be individualized by your veterinarian, but general IRIS-aligned starting points are:
- Healthy senior cats (screening): annual bloodwork including SDMA from age 7, every 6 months from age 10–12
- Stage 1–2: recheck bloodwork, urinalysis, and blood pressure roughly every 3–6 months
- Stage 3: recheck roughly every 2–3 months, or sooner if signs change
- Stage 4: close monitoring, often monthly or more frequently depending on stability and treatment response
Prognosis: What to Expect
Prognosis in feline CKD depends heavily on the stage at diagnosis, the rate of progression, and how well substages (proteinuria, blood pressure) are controlled:
- Cats diagnosed at stage 1–2 often live comfortably for years with appropriate diet and monitoring, particularly when hypertension and proteinuria are well controlled.
- Cats at stage 3 generally have a more variable course; many remain stable for a meaningful period with active management, though quality-of-life management becomes more central.
- Cats at stage 4 require intensive management and generally have a shorter, though individually variable, prognosis.
As with HCM, the biggest lever you have is catching and staging CKD as early as possible — this is the entire rationale for proactive senior-cat bloodwork rather than waiting for symptoms to appear.
Frequently Asked Questions
Can chronic kidney disease in cats be reversed?
No — CKD involves permanent loss of functioning kidney tissue and cannot be reversed. It can, however, often be slowed significantly with early detection, appropriate diet, and stage-appropriate treatment.
How long can a cat live with CKD?
It varies enormously by stage at diagnosis and how well the disease is managed. Many cats diagnosed early (IRIS stage 1–2) live comfortably for years; cats diagnosed at stage 3–4 generally have a shorter, more individually variable course.
What is SDMA and why does it matter?
SDMA (symmetric dimethylarginine) is a blood biomarker that can detect declining kidney function earlier than traditional creatinine testing, often catching CKD before a cat shows any symptoms — which allows treatment to start earlier, when it can do the most good.
Does dental disease really affect the kidneys?
Yes. Chronic dental and periodontal infection creates ongoing low-grade inflammation and bacterial exposure that the kidneys must help clear, and it's increasingly recognized as a contributing driver of progressive kidney decline in cats — which is why dental health is part of a complete CKD prevention and management plan.
What diet is best for a cat with CKD?
A veterinary therapeutic renal diet — controlled, high-quality protein and restricted phosphorus — is one of the most evidence-supported interventions for slowing CKD progression, generally recommended from IRIS stage 2 onward. The right diet and transition plan should be set with your veterinarian based on your cat's stage and preferences.
Is high blood pressure connected to CKD in cats?
Yes, closely. Hypertension can both cause and result from CKD, and uncontrolled blood pressure accelerates kidney damage as well as risking damage to the eyes, heart, and brain — which is why blood pressure is checked at every CKD recheck.
What is the AIM protein treatment for cats?
AIM (apoptosis inhibitor of macrophage) is a protein that cats have a species-specific defect in, discovered by Dr. Toru Miyazaki's research team, which is believed to be a major contributor to feline CKD susceptibility. Are HCM and CKD related in cats?
They frequently co-occur in senior cats and can each worsen the other — kidney disease affects fluid and blood pressure regulation that the heart must manage, and heart disease can affect blood flow to the kidneys. Yes. CKD is frequently silent until a meaningful amount of kidney function is already lost. Annual bloodwork including SDMA from around age 7, and more frequently in seniors, gives the best chance of catching CKD at the earliest, most manageable stage.
